Ionising radiation has long occupied a fraught space in medicine: feared, misunderstood, and often avoided, particularly in pregnancy. Much of this caution stems from historical data derived from early animal studies and nuclear disasters, which entrenched the belief that any exposure in utero carries significant risk. Yet, as medical imaging has evolved and become integral to modern diagnostics, so too has our understanding of radiation safety.



In reality, pregnant patients frequently present with medical and surgical conditions where timely and accurate imaging is essential. Delayed or withheld investigations may pose far greater risks than the radiation itself. Contemporary evidence demonstrates that the fetal doses from most types of diagnostic imaging are well below thresholds associated with deterministic harm, and the absolute increase in long-term stochastic risk remains very small.



This creates a familiar clinical tension: balancing theoretical fetal risk against very real maternal harm. In this context, imaging in pregnancy is not merely a clinical decision but an ethical one. Clinicians are frequently required to navigate risk–benefit discussions, counsel patients effectively, and make decisions that prioritise both maternal wellbeing and fetal safety. Understanding when not to image may be important, but understanding when imaging is necessary is critical.
